What is your wand made of?

What is your favorite mystical creature?

What do you look like?

What's your personality?

Which house do you wish to be in?
(Gryffindor, Hufflepuff, Ravenclaw, or Slytherin)
Please state your first, second, and third choice because
you may not get into that house. Don't get sore.
It's just that we're trying to balance the number of students in each house.

FOR YOU TO BE IN THAT HOUSE!
